Fairfax Orthopaedics staff are interested in speeding up the waiting process for our patients. If you are a first time patient, you may print the "Patient Registration" and "New Patient History Form" and fill them out and bring them with you.

There is also a space for your signature on the Patient Registration form that states that you have received, read and understand our HIPAA policy (privacy policy). You may either read this policy by clicking here prior to signing or you may opt to wait to sign that section until you are in our office where you may obtain a copy.

Returning patients may print the "Returning Patient History Form" and complete and bring it with you.

Foot and knee pain patients that run are asked to bring their running shoes. Read more
